【疫情的時候能看牙醫嗎?】

這是我最近最常收到的私訊問題

所以在這裡我參考了美國疾病管制與預防中心的【牙科治療指引】，整理出如何安全看牙的簡單說明，希望對你有幫助

通常會這樣問是害怕看牙時的感染風險，但同時也擔心延後牙齒治療會造成更嚴重的問題：例如蛀牙感染造成蜂窩性組織炎、牙齒外傷、牙齒缺損導致口內力量失衡．．．等，越拖越嚴重，結果導致狀況惡化到需要到醫院急診，反而增加醫療負擔與感染風險。

【牙科治療指引】內容蠻多且複雜的，我整理成以下兩大重點：

▐ #如果不得已非得要看牙，自己要注意什麼
​⠀⠀
1. 最基本的：
​⠀⠀- 全程佩戴口罩
​⠀⠀- 接受體溫量測
​⠀⠀- 接觸史評估
​⠀⠀- 候診時維持適當社交距離
這些我就不多講了
​⠀⠀
我最常被問的問題是：但看牙的時候無法戴口罩阿？
關於這點，美國那邊的作法是跟醫師問問題討論的時候口罩要戴回，只有在治療中才拿下能口罩

2. 陪病的人越少越好
最好是自己來，最多只能一位，減少群聚風險。
​⠀⠀
​⠀⠀
3. 術前使用抗微生物漱口水
如：chlorhexidine gluconate、povidone-iodine 漱口，降低診療過程中飛沫微粒帶有微生物的量，這通常院所會提供。

​⠀⠀
▐ #如何選擇感染風險較低的牙醫院所
就診時，你可以觀察以下幾點：
​⠀⠀
1. 首先，最基本的是醫療人員全員必須有全套的防護設備、防疫 SOP 與自主健康管理，以免跟患者交叉感染
​⠀⠀
​⠀⠀
2. 以全預約制為優先，且建議準時前往院所，不要提早到達也不要遲到，以減少在院內等候群聚的情況
​⠀⠀
​⠀⠀
3. 獨立式診間， 1 次僅診療 1 名病人，且診療後會進行【靜置後的全面消毒】與【通風】
​⠀⠀
關於這點，【通風】的重要性在最近被大幅討論：
美國疾病管制局修正了對於新冠疫情的指導方針，其警告，氣溶膠傳播是可能的，極微小的漂浮飛沫可能會傳播的更遠更久，如果在密閉空間內濃度會更高。這也被懷疑是之前有牙醫師全副防護看診仍染疫的可能原因。

4. 使用到噴霧器械（洗牙機、磨牙手機）時，會增加空氣中的飛沫，一般會建議減少使用。但如果沒辦法，真的需要使用噴霧式器械時，院所必須搭配高真空吸引（high evacuation suction）的設備或橡皮障防濕裝置（dental dams）等方式來減少飛沫

1. 我們進行了院內醫療人員的全面快篩，只有陰性的員工得以進入醫療院所

2. 醫師與助理的防護裝備、流程、消毒皆根據美國 CDC 標準進行

3. 一直以來，我們都是全預約制的獨立診間
​⠀⠀

【うがい薬】ポビドンヨード 殺菌消毒


「明治うがい薬」は「のどのうがい」だけではありません。

「ポビドンヨード」とは？
ポビドンヨード（Povidone-iodine）は、ヨウ素の酸化作用を利用した抗微生物成分です。
ヨウ素のアルコール溶液であるヨードチンキなどでは人体への刺激が強かったため、ポリビニルピロリドンとの錯化合物として合成されたポビドンヨードが現在では広く普及しています。
また、ポビドンヨードの独特な茶色は消毒作用の活性を示しています。手洗いとして使用した場合、消毒範囲も目で見てわかるので、安心です。

殺菌力はもちろんのこと、即効性に優れていることから、うがい薬や手指の殺菌、傷の消毒薬などに広く使われている代表的な消毒剤です。また、ポビドンヨードは人体や環境にもやさしい消毒剤です。

明治うがい薬の有効成分ポビドンヨードは、殺菌消毒剤の影響を受けにくいと言われている微生物や細菌に対しても効力を発揮します。その効力に着目したアメリカ航空宇宙局(NASA)は、1969年夏、宇宙船アポロ11号が人類初の月着陸を成し遂げて太平洋上に着水した際、地球外の未知の世界から運んでくるかも知れない細菌の洗浄・消毒にポビドンヨードが有効と判断し、更に海洋に対する汚染への配慮からポビドンヨードの殺菌消毒剤を船体に注ぎかけ洗浄・殺菌しました。

［うがいのすすめ］
飛沫感染とは、くしゃみやせきなどで空中に飛び散ったウイルスを吸い込んで感染することで、脅威的な伝染をします。感染力が強いインフルエンザウイルスは飛沫感染です。身近にかぜをひいている人がいなくても電車の中など人がたくさんいる場所では、油断は禁物です。しかし、ウイルスを吸い込んでしまうとすぐに症状が出る、というわけではありません。ウイルスが細胞内に入り込んでしまう前に洗い流すことが重要なのです。外出先から帰ったときは、まずうがいをし、のどの粘膜についたウイルスを洗いながしましょう。

正しいうがい液の作り方
１付属の計量カップで用法・用量を守って使用します
２計量カップを傾け、計量カップの2本の目盛(2〜4ml)の間までうがい薬を入れます
３計量カップを水平にし、線まで水を入れます
約60mlのうがい液が出来上がります

正しいうがいの仕方
（正しい濃度に薄めたうがい液約60mlを３度に分けてうがいをします）
１.食べかすや、口の中の有機物を取るため口に含み比較的強くうがいをします（１度目のうがい）
２.しっかり上をむいて、のどの奥までうがい液が届くように15秒程度うがいをします（２度目のうがい）
３.２度目のうがいと同様、うがい液が口に届くように15秒程度うがいをします（３度目のうがい）
