Wellll... I was already thinking to myself - “Daaaangggg here we go, a different kind of paddle of shame back to the boat” thanks god for leashes! hahaha!!!

浪卷浪快，我動作慢了一點，一上浪馬上就被卷下去。洗衣機模式才開始幾秒我就發現，感覺不對喔！我的褲子呢？完全沖掉了！朋友叫我快從珊瑚區划出來因為外面又有set浪進來，可是我只在那邊揮手說「不！我的褲子不見啦！」他們沒聽懂，我就繼續滾滾滾，然後摸到板尾的時間就突然找到掛在腳繩上的丁字褲😂 幸好有腳繩🙈 不然當下我心裡只在想「完了，完了！我這樣要來個 paddle of shame 才能回船上」😂😂😂 #衝浪的樂趣

【2016敗血症指引-中文摘要】

現在的學生都很好命，老師怕學生讀67頁的原文太累，於是老師自己先讀一篇，再把精華的建議全部翻成成中文。只有短短六頁的中文，這樣要學生們讀他們就不會生氣了。

和醫界同好們分享，請自行下載典藏。

------以下為重點及與2012有差異之處------

- 強調應於三小時內給予至少30 mL/kg的crystalloid fluid。
- 強調於初始復甦後應持續評估血行動力狀態，並持續輸液治療。
- 輸液復甦的依據：動態指標優於靜態指標。
- 醫院系統應對敗血症有績效提升計劃。
- 不應持續使用全身性抗微生物製劑來預防感染。
- 強調一開始用廣效性，儘早改窄效性的抗微生物製劑治療策略。
- 應依PK/PD原則及個別藥物屬性來調整抗微生物製劑之劑量。
- 強調對敗血性休克病人應使用empiric combination therapy；Sepsis without shock、neutropenic sepsis、bacteremia則不建議常規使用combination therapy。
- 某些病人可使用比7-10天更短的微生物製劑療程，特別是其臨床狀況在有效的感染源控制後快速改善的腹腔內感染或泌尿道感染。
- 強調使用procalcitonin levels來輔助決定是否縮短或停止抗微生物製劑治療。
- 強調輸液治療時crystalloid較HES及gelatin好，crystalloid中balanced crystalloid（如Ringer’s Lactate）和saline沒明顯差異。
- 強調升壓劑首選用norepinephrine，二線可選epinephrine或vasopressin。
- 強調若輸液及升壓劑即可穩定血行動力，不應使用IV hydrocortisone。
- 強調Hb < 7.0才輸RBC（除非有心肌缺氧、嚴重低血氧、或急性出血）。
- 呼吸器方面，建議Vt = 6 mL/kg（包括無ARDS病人）、Pplat < 30 cmH2O、higher PEEP、RM、Prone、NMBA、conservative fluid strategy、床頭搖高、SBT、weaning protocol；不建議HFOV、β-2 agonists、routine PA catheterization。
- 強調減少鎮靜劑使用。
- 有A-line之病人測血糖時應使用arterial blood。
- 若僅是creatinine上升或oliguria而無其他明確的透析適應症，不建議開始透析。
- 預防靜脈血栓時，LMWH優於UFH。
- 強調沒有胃腸道出血風險之病人，不建議給予壓力性潰瘍之預防。
- 強調可由腸道灌食之病人應儘早開始腸道營養。
- 不建議在七天內開始非腸道營養，無論是單獨使用或合併部份腸道營養。七天內給予iv glucose及儘可能的腸道營養即可。
- 不建議常規監控胃殘餘量，除非是對灌食耐受不佳或嗆食高風險之病人。
- 灌食耐受不佳時建議使用prokinetic agents或置放post-pyloric feeding tubes。
- 強調訂定「照護目標」的重要。
